For prizes between $600 and $5,000, you can either claim them by mail or at a lottery office. If you elect to claim in-person then you will need to bring your original winning ticket, signed on the back, as well as a valid form of photo ID, and a completed Claim Form.
If you win a prize above $5,000 then this will need to be claimed in person at your nearest lottery office. You are required to bring the original winning ticket, a valid form of photo I.D., and a completed Claim Form.
For prizes over $510,000, you must claim your winnings at the Louisiana Lottery Headquarters, located in Baton Rouge. You will need to bring the original ticket, a valid form of photo I.D., and a completed Claim Form.

The Louisiana Lottery is required to withhold 24% for federal taxes for prizes above $5,000, with an additional 3% for state taxes. Any winnings above $600 should be reported to the IRS and the Louisiana Department of Revenue and Taxation.
