Before you do anything else, remember to sign the back of your winning ticket!
For prizes won up to $600, you can claim at and Kentucky Lottery retailer without the need for a claim form.
If you have won a prize between $600 and $5,000, you will need to print off and complete a prize claim form. With this, you can either visit an authorized Cashing Agent or Super Cashing Agent, or you can claim your winnings at select retails who can redeem prizes up to $5,000.
For prizes between $5,000 and up to $25,000 please visit an authorized Super Cashing Agent, or the Kentucky Lottery Headquarters in Louisville.
Any prizes won that are over $25,000 will require you to go to the Kentucky Lottery Headquarters in Lousiville.
For any claims by mail, please send the required information to the following address. You will need to include a completed Prize Claim form, your winning ticket with the signature on the back, and a copy of a valid government issues photo I.D.
For Kentucky lottery prizes that are over $5,000, the Kentucky Lottery is required to withold an initial 24% of the amount in federal tax, as well as an additional 4.5% in state tax. It should be noted that some circumstances will mean that the tax rates will be higher or lower.
